What decision or review will this evidence support? Define the purpose before adding another record or field.

Make ownership visible

Agree who creates the record, who reviews it and who follows up. A document without an owner rarely changes how work happens.

Connect evidence to action

Keep the sequence clear: requirement, activity, evidence, review and action. Check whether the agreed action was completed and whether it addressed the issue.
